Spain has successfully broken the taboo surrounding mental health, encouraging more people to seek help for conditions that were once kept hidden. However, this increased openness has exposed a severe structural problem. The country currently has only six clinical psychologists per 100,000 inhabitants, compared to an average of 18 across Europe. This shortage creates long waiting lists within the public health system and forces many patients to turn to private care. As a result, access to psychotherapy increasingly depends on personal income rather than clinical need. Those who can afford it pay out of pocket for timely sessions. Those who cannot must wait months for limited public appointments or go without treatment entirely. The gap between rising awareness and actual capacity has produced a two-tier system. Experts warn that without significant investment in public mental health staffing, the progress made in reducing stigma will not translate into real care for most citizens. The paradox is clear: more people are ready to ask for help, but the system is not ready to answer.
